Illness representation after acute myocardial infarction: impact on in-hospital recovery.

BACKGROUND Despite significant progress in the treatment of coronary artery disease, myocardial infarction is still the leading cause of death in the United States. As suggested by Leventhal's Self-Regulation Model of Illness, the continued high morbidity and mortality may be due to a failure to address the role of psychosocial factors such as illness representation, depression, and anxiety in ...

CRITICAL CARE/RESPIRATORY CARE Corticosteroid Replacement in Critical Illness

Intensivists routinely administer glucocorticoids, which are potent anti-inflammatory, immune, metabolic, and adrenergic modulating compounds at physiologic and pharmacologic doses. Recent investigations report increasing recognition of relative or absolute adrenal insufficiency in critically ill patients and the impact of cortisol responsiveness as a predictor of outcome for the critically ill...

Peer Support as a Novel Strategy to Mitigate Post-Intensive Care Syndrome.

Post-intensive care syndrome, a condition defined by new or worsening impairment in cognition, mental health, and physical function after critical illness, has emerged in the past decade as a common and life-altering consequence of critical illness. New strategies are urgently needed to mitigate the risk of neuropsychological and functional impairment common after critical illness and to prepar...

During critical illness the gut does not pass the acid test

The composition and function of intestinal microflora are emerging as integral to both health and disease. During critical illness the normal microbiota are rapidly replaced by pathogenic species as a result of both the physiologic stress itself and the use of antibiotics. In this report, the authors use fecal pH as a surrogate marker to determine the predictive value of the functional output o...

A path to precision in the ICU

Precision medicine is increasingly touted as a groundbreaking new paradigm in biomedicine. In the ICU, the complexity and ambiguity of critical illness syndromes have been identified as fundamental justifications for the adoption of a precision approach to research and practice. Inherently protean diseases states such as sepsis and acute respiratory distress syndrome have manifestations that ar...
